The WMS Band Forum is an educational portal created and maintained by the Wellsburg Middle School Band Director (Mr. Timothy Turner). Only band students and their families are allowed to register/login to the Forum. The Forum is a closed and monitored platform. The Forum is used for educational purposes and includes tools that allow the director, parents and your child to communicate, share and collaborate with other members of the Forum. Students will gain social networking skills and be able to share documents and opinions, post comments and collaborate with other students.
I understand that my child will be able to communicate with other members on the Forum, and that my child could participate in online learning activities that will allow him/her to share documents and opinions, post comments and collaborate with other students. I understand that my child will be able to create a profile accessible by other members of this Forum.
I hereby give my permission for my child, named above, to obtain a Forum username and password and to use the Forum in and out of the classrooms of Wellsburg Middle School. At any time, a parent may revoke his/her consent, and refuse to allow their child to further use the Forum, and request that the director delete the information.

This form only needs to be filled out one time. Once a student moves on to high school, he/she will be able to view the Forum and post comments, but will not be able to contact other members through the Forum.
