Pictures, podcasts, and/or videos will be posted on the WMS Band Homepage http://www.wmsband.net that may contain your child's image. In compliance with the West Virginia State Board of Education Policy 2460 (Acceptable Use of the Internet) Section 126-41-3.4.5c, the most information given about the students may be his/her first name. Please don't let your child miss out on such an exciting opportunity. Please print out, sign and return this permission slip to Mr. Turner as soon as possible.

I hereby give my permission as parent of ______________________ (print) for the image, voice, or video of my son/daughter to be included on the web site of the WMS Band. Only the first name of my child may be given on the site with the image. I understand that the Brooke County Board of Education and its employees assumes NO responsibility for any unforeseen harm to privacy.
